PATIENT EDUCATION

During Your Visit

  • Please arrive 20 mins prior to your scheduled appointment, to allow completion of paperwork.
  • If you need to cancel or reschedule your appointment please call our office, within 24 hour.
  • A minimum charge of $20 will be billed to all (No-show) patients.
  • Please have your insurance card and a photo I.D. present.
  • All payments such as: (Self pay, Co-pay, or Co-insurances) fees will be collected at the time of your visit.
  • Please bring a list of your home medications, and a copy of any prior health records, including lab results, Xrays.

New Patients

  • Welcome
  • In order to make your first visit as informative as possible please follow the directions below.
  • Please arrive in the office 20 minutes prior to your scheduled appointment time and bring the following items with you:
    • Insurance card.
    • Valid driver's license or another valid form of ID.
    • Completed Patient Registration form and Patient H&P form. You can download and print these forms form our website.
    • List of medications.
    • Copy of previous health record.

Returning Patients

  • If you are a returning patient to our clinic please consider the following items that will make your visit more efficient:
    • Please present your insurance card to the front desk at each visit as well as your copay and any outstanding payments on your account.
    • Inform the front desk regarding any changes to your address and contact information.
    • Inform the staff and the physician if there have been any changes to your medical or surgical history since you were last seen.
    • Bring a complete list of your medications, or your medications themselves to each visit so we can update our records.
    • Be sure to request all your prescription refills at the time of your visit. Also indicate whether you require 30 or 90 day supplies.
  • After the visit if you require any results to be faxed to your primary care or other physicians, please provide us with the name, phone and fax number of the physician.

Labs and Xray

  • At time of your visit, please inform us if your insurance requires you to utilize a specific Laboratory.
  • We order Lab work from the following lab:
    • Regional Lab Outreach, Service Centers Located at: Medical Services Building (201 Talmadge Drive), Resource Medical Office Park (1500 Ogelthorpe Avenue, Suite 3700).
  • Radiology
  • We refer our patients to Athens Diagnostic Center for Radiology needs. It is located at 1063 Baxter Street, Athens GA 30606.
